The "T" stands for transgendered, but transgendered people tend to not only be abused by the heterosexist system, but also neglected by the LGB community.  While the LGB community focuses on the fight for marriage, transgendered people struggle to be allowed to use restrooms.  Transgender people face discrimination in nearly every aspect of their lives, but it goes unnoticed by most. More than a quarter of trans people interviewed say they have lost a job to discrimination, and 90% of them claim to have been harassed at work. Plus, many health insurance plans only cover procedures for one gender, so trans people can't get the procedures they need.  Honestly, I don't know what to do at the moment. I'm just trying to raise your awareness.

Something you could do, is avoid using offensive words for transgendered people. "Heshe" and "shemale" are insulting. So is trann with a "y" at the end. A lot of people aren't aware of that, but it has a long history of being used to objectify trans people.

Transgender is an umbrella term for people who identify with a different gender than their biological s-x. It doesn't only apply to people who have had surgery.
