1. The first problem is the biggest, give a plot for the roleplay. A normal plot for fantasy would be a supernatural school. I say that's too common. You could do this plot
Plot #1: a few kids live on different parts of earth. They would have never known each other if a giant meteor crashed ont earth, shattering on contact. The meteor spewed powers that had once belonged to a king on a planet far off, each child received a power, and a flaw. Every superhero has a villain. The king who had his powers stolen made a villain for each child. Will life ever be the same for these children? ( in this one you would have to make a villain for your character, a powerful one. )
Plot#2: everyone knows of the school that almost gets destroyed but the hero saves the day. Not in this world. An evil headmaster dominated the school, sending all the supernaturals into fear, and dark, deadly quests. The school has changed. You get sent on a quest to fetch the dragons fortune on the Red Abyss. You may not succeed but even if you do, you can not come home a hero. But you do gain freedom from the school. The problem ism no one has ever survived a quest. Is you escape from the quest track and run, the headmaster will get you. Tack you. #### ###. Will you survive?
You can make up another one.
2. The forms are also a problem, but not big, you should add weapon or fatal flaw. This is required for certain people who don't understand powercaps. Your forms are fine, just saying, you should add some stuff.
3. You should change the title according to the plot, a name like that will not attract many good Roleplayers. I only came cause I am crazy and kinda bored. :3
