Use Skurge to get it from across the hallway on the first floor.
Buy it for 30 beans from Fred/George's shop.
Found in the attic of Borgin and Burkes, also in the hidden passage behind the curtain closest to the library on the second floor.
Lock up all the imps in Borgin and Burkes.
Collect all 100 other cards and meet Dumbledore outside his office on the third floor.
Crawl under the bookshelf near the Gryffindor portrait on the seventh floor and use Alohomora to get this card.
